UHL provides a Birth certificate request form also. The form is usually filled up by the parents (mother/father).
Generally, a Birth Certificate is distributed after three days of request. The mother or father can receive the birth certificate from the filled medical department. If the parents are absent, you need a legal authorization letter.
You will also need the General Diary (GD) entry copy from the local police station for the Duplicate Birth Certificate. Parents must submit the application form and charge BDT 500 to the filled medical department.
Modification of Birth Certificate requires an original copy of the Birth Certificate, application form, and charge of BDT 500.
The Blood Bank of United Hospital Limited is government-approved. It aims to provide patients with the safest possible blood, equal to any international standard.
All gratuities received in the blood bank are voluntary non-remunerated. During a blood transfusion, UHL takes all necessary safety measures, including conducting compulsory screening tests for transmissible diseases for both the donors and the recipients. At COVID times, many people don’t want to visit labs to avoid the spread of contamination.
Some patients prefer to get the blood sample collection at home. Older persons face problems in traveling and choose the tests to be supervised in comfort at home. In addition, weak or paralyzed patients cannot move or go to the lab to perform a test or when it is not suitable for them to reach the lab.
